These Terms & Conditions ("Terms") govern your use of limbehavioral.com (the "Website"), operated by LIM Behavioral Health ("LIM Behavioral Health," "we," "us" or "our"). By using the Website, you agree to these Terms. If you do not agree, please do not use the Website.
Website information is not medical advice
Content on the Website, including service descriptions and blog articles, is provided for general educational purposes only. It is not a substitute for professional diagnosis, treatment or advice, and it should not be relied on to make decisions about your health. Always speak with a qualified healthcare provider about your individual situation.
Not for emergencies
The Website, its forms and our general phone and email lines are not intended for emergencies. If you are experiencing a mental health crisis or thinking about harming yourself or others, call or text 988 (Suicide & Crisis Lifeline), dial 911 or go to the nearest emergency room.
No provider–patient relationship through the Website
Using the Website, submitting a form or contacting us does not create a provider–patient relationship. A provider–patient relationship begins only after you complete our intake process and attend an initial appointment with one of our clinicians. Submitting a request does not guarantee an appointment, a particular clinician or any specific treatment. All treatment decisions are made by your clinician based on a clinical evaluation.
Eligibility and telehealth
- We provide care to patients aged 10 and older. Requests for patients under 18 must be submitted by a parent or legal guardian.
- Our clinicians are licensed in Texas, Washington and Maryland. For telehealth visits, you must be physically located in one of these states at the time of your appointment.
- In-person visits are available at our Dallas, Texas office. Telehealth may not be appropriate for every situation, and your clinician may recommend an in-person visit.
Appointments, cancellations and payment
- Online scheduling is provided through Tebra, a third-party platform with its own terms of use and privacy policy.
- Please give at least 24 hours' notice to cancel or reschedule an appointment so the time can be offered to another patient.
- You are responsible for confirming your insurance coverage and for any copayments, deductibles or self-pay fees. Insurance plans listed on the Website are provided for convenience and do not guarantee coverage for a particular service.
Acceptable use
When using the Website, you agree not to:
- Provide false or misleading information, or submit information on behalf of someone without their authorization.
- Use the Website for any unlawful purpose or in violation of these Terms.
- Attempt to gain unauthorized access to the Website, its systems or any accounts.
